You have received a bill from us because you or the patient (stated on the statement) was provided services by a behavioral health provider of MedOptions in a skilled nursing or assisted living facility.

If you are a MEDICAID patient or have additional insurance coverage, you may not owe the amount shown on this statement. Please contact us at (866) 916-8882 or email to make any updates to your insurance or contact information.

To pay your bill, please choose one of the following options:

  • Pay online: please click on the link below corresponding to the date of your statement. Once redirected to the appropriate portal, you will need to provide the Statement ID and Passcode included on your statement
    • For statements dated prior to February 15, 2021: click here
    • For statements dated on or after February 15, 2021: click here
  • Pay by mail: send in your check along with the payment coupon on the bottom of your statement
